How it Works

Using Insurance

On the day of your appointment

 

We will collect your copay at the beginning of your visit.  For most insurance plans, you will be responsible for your office visit copay.  Michigan Avenue Primary Care accepts the following forms of payment:

  • Cash

  • Check

  • American Express

  • Mastercard

  • Visa

  • Discover

Please bring your insurance card with you on the day of your visit.  If you do not have your insurance card, you have several options:

  • Contact your insurance company to find out how to obtain a temporary replacement card.

  • Pay for your visit and submit a claim to your insurance company for reimbursement.

  • Leave a credit card on file with us for up to 24 hours after your visit until you are able to obtain your insurance information.

  • Ask a family member on your same insurance plan for the billing information.  We will need:

    • ID #

    • Group#

    • Address to Send Claims

    • The Policyholder’s name, date of birth, SS#, and your relationship to that person

After your appointment

 

We will submit a claim to your insurance company for all services rendered.  You will receive a statement from Michigan Avenue Primary Care after your insurance has processed the claim.  Often, you will receive a statement within 20-30 days of your visit with us.  However, your insurance company may take longer to process the claim, which will delay the timeframe within which you receive your statement. 

Not using insurance

 

We understand that medical care can be expensive.  Our intent is to provide as much information as possible and avoid any unexpected surprises regarding payment.

 

We offer a 35% discount on all fees for self-pay patients.

  • Full Payment will be collected at the end of your visit.  We accept:

    • Cash

    • Credit/Debit Card (Visa, Mastercard, Discover, & American Express)

    • Check

  • Doctor consults (which do not include individual tests and treatments) can range widely depending on the extent of the visit.

    • New patients: $200-$500

    • Established patients: $100-$350

  • Costs for individual tests and treatments are added to the cost of the doctor's consult.

    • Xrays: $90-$200

    • Blood Tests: varies widely, depending on the test(s) ordered

    • Dispensed medication: $10-$20

    • Injections: $10-$50

    • Procedures: $150-$500

  • We can provide estimates for your visit, but cannot predict precisely what the total charge will be.

  • You can discuss treatment and testing options with both our Medical Assistants and Doctors over the course of your visit.

In order to provide the highest quality medical care for you our patients, we must occasionally evaluate our prices. You will find our current, updated pricing  to be competitive with other offices in the surrounding area. New Prices are effective 8/1/2011.

Additionally, we will accept any other PPO plan not on the above list.  However, if we are not in-network with the PPO (not one of the companies listed above), the patient may be responsible for a larger portion than if they see a physician that is in their network.

 

HMO Plans
The patient must contact their insurance company to select Dr. Niby Matheew or Dr. Joshua Merok as a primary care provider prior to scheduling an appointment for insurance to cover the visit.

 

If you have specific questions about your coverage please contact your insurance company. We suggest contacting your insurance company’s customer service department by calling the number located on your card to help you locate a provider.

 

Contacting your Insurance

Each insurance plan is different and your insurance company representative will be able to provide you with plan-specific information.

 

You can contact a customer service representative at your insurance company by calling the (800) number on the back of your insurance card.  If you’ve lost your insurance card, your Human Resources department or your school’s health services will also have this contact information.
